Adult Rehabilitative Mental Health Services, also known as ARMHS, is a community
based service that helps
adults with mental illness manage their symptoms while living in the community.
ARMHS teaches, develops, and enhances skills needed to live a healthy life.

Our Services
Adult Rehabilitative Mental Health Services (ARMHS)
Eligibility Criteria
To qualify for ARMHS, a Minnesota Health Care Program (MHCP) member must:

Be 18 years or older
-
Have a primary diagnosis of mental illness confirmed by a Diagnostic Assessment
-
Have a completed Level of Care assessment
-
Showing significant functional impairments in three or more areas from the Functional Assessment
Basic Living and Social Skills (BLSS)
We service and assist members in developing daily living skills essential for independent functioning. That includes:

Communication and interpersonal skills
-
Community engagement
-
Crisis and relapse prevention
-
Budgeting, nutrition, and shopping
-
Transportation
-
Medication management and symptom monitoring
-
Household and employment-related skills
-
Transitioning to independent community living
Certified Peer Specialist (CPS) Services
CPS services offer non-clinical, recovery-focused support:

Encouraging empowerment and self-direction
-
Addressing stigma, loss, and life transitions post-hospitalization
Community Intervention
Strategies delivered on behalf of a member to:
-
Reduce barriers to community integration
-
Prevent hospitalization or institutionalization
Key Guidelines:
-
Directed solely toward the member’s treatment
-
Delivered individually, not in groups
-
Can be face-to-face or via phone, with documentation
-
Member need not be present if intervention is more effective otherwise
Functional Assessment (FA)
A comprehensive narrative assessment of how mental health symptoms affect daily life and functioning.

This includes non-mental health factors impacting quality of life.
-
Level of Care Assessment determines the appropriate intensity of care.
Used to inform service referrals and planning.
Individual Treatment Plan (ITP)
A written, client-centered strategy developed post-assessment and prior to service delivery. It includes:

Diagnostic and functional baseline
-
Goals, objectives, and responsible parties
-
Recovery vision, strategies, and culturally appropriate interventions
-
Medication Education
This service educates our clients on:
-
Mental illness and treatment medication
-
Side effects and proper medication usage
Transition to Community Living (TCL)
T.C.L. supports re-entry from higher levels of care.

Reestablishing provider-member contact pre-discharge
-
Implementing discharge plans from treatment centers or hospitals
Progress Notes
Every ARMHS session must be documented with a detailed progress note including:

Service type, date, time, and location
-
Interventions used and objectives addressed
-
Member’s response
-
Future plans and any service changes
-
Provider’s name, credentials, and signature
Noncovered ARMHS Services

Transportation
-
Services by non-enrolled providers
-
Volunteer services
-
Household chores (laundry, housekeeping, shopping)
-
Time not actively delivering services (e.g., on-call)
-
Recreational or social activities not tied to rehab
-
Job-specific training or case management activities
-
Outreach to prospective clients
-
Room and board
